2008-2009 Secondary Application



- On February 13th the Rockford School Board approved a new Zone Assignment Plan based on zones for middle schools and high schools. The zone plan requires 6th, 7th, 9th and 10th grade students to attend their Attendance Zone schools. The plan will continue to phase in a grade a year until fully implemented.

- 5th and 8th grade students currently enrolled in Rockford Public Schools received a letter in the mail notifying them of their assignments for 6th and 9th grade to their Zone school. If your child did not receive a letter, please the Family Resource Center at 815-967-8070 to verify the address on file and to find out what your child's assignment will be.

- Students who are New to the District and enrolling for grades 6-12 for the 2008-2009 school year must visit the Family Resource Center to fill out an application and to provide required paperwork.

Students entering 6th, 7th, 9th, or 10th will be assigned to their Zone school.

Students entering 8th, 11th, and 12th will be assigned under the "Choice" assignment plan which means parents can select a school assignment based school choice and seat availability.

Parents must bring two proofs of address and original birth certificate.

PROOF OF RESIDENCY*

ONE OF THE FOLLOWING:

• Mortgage Closing Papers (homeowners)(intent to purchase will not be accepted)

• Signed and dated lease and proof of last month’s payment, e.g. canceled check or receipts (renters)(must be within last 30 days)

OR

TWO OF THE FOLLOWING:

• Most recent property tax bill and proof of payment, e.g. canceled check or Form 1098(homeowners)

• “Letter of Residence” to be used when the person seeking to enroll a student is living with a District resident (form available at the Family Resource Center)

• Utility Bills (must be within last 30 days)

• Current DHS Medical Card (must be within last 30 days)

• Home visit verifying residency documents.

• Mail received at new residence, including forwarded mail (only ONE of these will be accepted)


*Drivers License, Rent Receipts and Voter’s Registration, etc. can be obtained under false pretenses and may NOT be used for verification of residency.


The office is currently taking walk ins but will begin making appointments when it gets busier in July and August. Please call first to find out if an appointment is necessary.


GRANDFATHER
Once an assignment has been made, a student may continue in the assigned school until he or she completes the highest grade offered at his or her assigned school.

SIBLING PRIORITY
For the purpose of student assignment, siblings are defined as students who have a common parent, legal guardian, or educational guardian and live in the same household. Sibling priority will apply only to the first choice school. Siblings of students attending a school are granted a priority to attend that school before other students, provided:

a. Space is available
b. Enrollment of the sibling is concurrent

PROXIMITY PRIORITY
Students whose home address is within 1.5 miles of their first choice middle or high school receive a proximity priority. Those students are assigned to that school before other applicants whose home address is more distant from the school. A distance of 1.5 miles around a school, based upon commonly traveled roadways, is considered the school's proximity zone.

ASSIGNMENT OF SEATS
Is based upon seat availability.

TRANSFERS
Requests for school transfers can be made by submitting a written appeal at the Family Resource Center. At this time only transfers that are being approved are:

If a student has sibling or proximity priority to a school if in grades K-5, 8, 11, and 12.


SPECIAL PROGRAMS
The student assignment procedures, which are applied to regular education students, will also be applied to special education students and bilingual students to the greatest extent possible. District adopted criteria will be used for assignment to the Gifted and CAPA programs.
