REQUIRED DOCUMENTATION
Bring ALL required DOCUMENTATION* which includes:
1) Original Birth Certificate (county not hospital) Also accepted: passport or visa
2) TWO acceptable proofs of residency as listed below.
ONE OF THESE:
Mortgage Closing Papers (homeowners)(intent to purchase will not be accepted)
Signed and dated Lease and proof of last month's payment, e.g. canceled check or receipts (renters)
(must be within last 30 days)
"Letter of Residence" to be used when the person seeking to enroll a student is living with a District resident
and the resident's proof of address as listed below (form available at the Family Resource Center)

TWO OF THESE:
Most recent Property Tax Bill and proof of payment, e.g. canceled check or Form 1098 (homeowners)
Current DHS Medical Card (must be within last 30 days)
Home visit verifying residency documents
Utility Bills (must be within last 30 days)
Mail received at new residence, including forwarded mail (only ONE of these will be accepted)
*Drivers License, Rent Receipts and Voter's Registration, etc. can be obtained under false pretenses and may NOT be used for verification of residency.
3) Transcript information from previous school (if applicable)
4) State required “In Good Standing” Student Transfer Form (if coming from another Illinois Public School)
5) Current Individual Education Plan (IEP) for student receiving special education services (if applicable)
6) Children who hear or speak a language other than English must accompany the parent for a language assessment. |
