This form is to be completed by the Substitute Teacher after every assignment and left in the teacher's mailbox.
This form is to be completed in the event a differential is split between multiple staff.
Complete this form and return with required documentation to H. R. Dept. Note: Name changes cannot be changed on any District documents (i.e., paychecks, insurance) without a copy of your new social security card and marriage certificate/divorce decree.
This form must be signed by a physician and verifies an employee's medical history and good health.
This form needs to be completed by professional staff request for Non-Emergency Leave before or after a holiday or first or last day of school.
This form indicates previous teaching record from other districts. Employees must complete the top section of the form and send to all previous employers.
This form must be submitted to claim an exemption for working in Illinois while residing in another state.
