| WHAT IS WORKERS' COMPENSATION? | Workers' compensation is a system of benefits provided by law to most workers who have job-related injuries or diseases. These benefits are paid regardless of fault. It is against the law for the employer to harass, discharge, refuse to rehire or in any way discriminate against an employee for exercising his or her rights under the Workers' Compensation or Occupational Diseases Acts.
WHEN AN ACCIDENT HAPPENS AT WORK STEP 1: Report the accident immediately to your supervisor.STEP 2: Seek appropriate medical attention.- First Aid...is this a "Band-Aid" injury?
- Go to the PHYSICIAN'S IMMEDIATE CARE with your supervisor, if medical attention is needed.
- Go to the Emergency Room for serious or castastrophic injuries.
- If you choose to see your own physician, please have him/her verify coverage with Sara Jones, Workers' Compensation Coordinator.
Bills and medical documentation must be sent to: Sara Jones, RN, MS. Workers' Compensation Coordinator Rockford Public Schools Human Resources Department 201 S. Madison St. Rockford, IL, 61104-2092 815-966-3908, Fax 815-966-3057 STEP 3: Report the accident to the Workers' Compensation Coordinator.
- Office telephone 966-3908, cell phone 815-742-4712, if accident has caused injury.
- Complete "Employee Accident Report" form .
- Fax completed form to Human Resources at 966-3057; mail copy to Human Resources.
STEP 4: The supervisor should request that all accidents be reported immediately.
- The supervisor should complete "Supervisor's Accident Investigation Report and send to the Workers'
Compensation Coordinator.
