Physical, Dental and Eye Exams & Immunizations
medicalitems State law requires that all children show proof of having a health examination and required immunizations by October 15th of the school year. A student must have a physical exam within one year prior to entering:

* pre-kindergarten program
* kindergarten
* sixth grade
* ninth grade
* any new student to the school district

Students must show proof of basic immunizations and needed boosters against Diphtheria, Tetanus, Whooping Cough, Polio, Red Measles, Mumps, and Rubella. Pre-school, kindergarten, first through sixth grade students must show proof of varicella(chicken pox) vaccine or having had the disease. Students entering pre-school must show proof of Haemophilus Influenzae Type B. Students entering pre-school and kindergarten must show proof of lead screening. The above lead screenings are required of students living in ZIP codes 61101, 61102, 61103 and 61104.

Students entering pre-school, fifth - twelfth grades must show proof of Hepatitis B immunizations. Students entering ninth grade must show proof of Tetanus and Diphtheria(Td) immunizations.

Dental exams are required for kindergarten, second and sixth grade students by May 15th of that school year.

Physical and dental exams may be scheduled at your family doctor’s office, family dentist office, Winnebago County Health Department(720-4000), Blackhawk Park School-Linked Health Center(972-7200) or Crusader Clinic(490-1600).

Children whose parents or legal guardians object to physical exams or to immunizations on religious grounds may submit a letter explaining religious beliefs and request an exemption.

Eye exams are required before October 15th of the school year for all children enrolling in Kindergarten and any student enrolling for the first time in a public, private, or parochial school.
